What is the chemical difference between baking soda and baking powder?

When developing precise cake recipes, why can you not simply swap one for the other without ruining the final product?

Baking soda is pure sodium bicarbonate; it requires an acidic liquid (like buttermilk or citrus) to activate and rise. Baking powder contains baking soda PLUS a powdered acid (cream of tartar). It activates instantly when it touches any liquid, and usually again when exposed to oven heat (double-acting).
